A handoff note needs four lines: who holds the responsibility, the clock time it changes hands, the one question still open and whose it is, and the release rule at the door. The fourth line is the one people skip. Pennsylvania's licensing rule puts it plainly: release only to the parent, or to an individual the enrolling parent designated in writing (Pennsylvania Code § 3270.117), so "Sam is getting her" is a plan only once Sam is on the list. Where no provider sits in the middle, the fourth line becomes the address and the contact method instead.
Write four lines, not the whole day
Responsibility: the specific nonclinical job. Staying in the house. Driving one leg. Being the phone contact between two named hours. "Can you take over" is four different jobs wearing one sentence.
Time and place: a start, a finish and an address, in clock time. Open question: the thing nobody has answered yet, with a name and a deadline beside it. Door rule: who is authorized to collect, and where that authorization is written down.
Leave the diagnosis, the record number and the history off the note. ChildCare.gov's page on provider policies shows what a program actually holds in writing, which is authorized adults, hours of operation, program schedule and closures, and payment and late fees (Your Responsibilities Once You Find Care). A note between two adults needs less than that, not more. Where professional instructions matter, ask the care team how they should be accessed rather than copying them out from memory.
Name the clock time, then name the gap
"I leave at three, you get there at three-thirty" is a 30-minute gap written as a plan. Put the gap on the note as its own line with a name against it, and resolve it before the earlier person walks out the door.
Work counts time in the same unit. The Department of Labor's fact sheet on counting FMLA leave says leave is used "in the smallest unit of time made available by their employer for other types of leave, as long as that smallest unit of time is not greater than an hour" (Fact Sheet #28I). An hour is the coarsest unit federal leave counting allows. A household handoff that only resolves to "afternoon" is coarser than the rule an employer has to meet.
The same fact sheet is useful for the other direction too: time somebody is not scheduled to report for work is not counted as FMLA leave (Fact Sheet #28I). Off-shift hours are not automatically available hours. An adult sleeping after nights, in class, or already driving somebody else is unavailable, and the note should say so rather than leaving the space blank.
An example: a 3:00 pm start and a 6:00 pm close
Take a published center as the example. Bright Horizons at University City in Philadelphia lists center hours of 6:30 AM to 6:00 PM, Monday to Friday, at 3600 Civic Center Blvd (center page). One adult starts a shift at 3:00 pm and leaves the house at 2:20 pm. The other finishes at 5:15 pm, 25 minutes from the center.
The note reads:
- Responsibility: collect at the center, then home. Jordan.
- Time and place: 5:40 to 6:00 pm, 3600 Civic Center Blvd. Center closes 6:00 pm.
- Open: Friday's return ride is unarranged. Sam calls the office before 4:00 pm Thursday and replies here.
- Door: Jordan is on the center's authorized list, added March 3.
Read the second and third lines aloud before leaving. Those are the two that get misremembered, and the 20-minute margin between 5:40 and the close is the whole reason the third line has a deadline on it.
Timestamp the note and delete the old one
Put the time of the last edit at the top. A printed sheet on the counter and an edited message on a phone are two versions of the same plan, and the person arriving will follow whichever one they see first. When something changes, correct the existing entry and send the change to the people it touches.
A sent message is not a read message, especially to somebody driving or on a floor where phones stay in a locker. Agree what acknowledgment looks like: a reply with the time in it, not a thumbs up. Standing arrangements belong on the shared calendar so the note only ever carries today's change, and the same discipline at work is the end-of-shift handoff. For a medical visit, the split into separate jobs is in appointment coordination.
Write these two lines before you go: "Responsibility passes at 5:40 pm at 3600 Civic Center Blvd, and the center closes at 6:00 pm. Open: Friday's return ride, Sam calls before 4:00 pm Thursday."
